By the maximum length of a single trip — the per-trip cap — not by how many times you travel. The three standard caps are 8, 15 and 30 days. At September 2026 measured rates (Worldwide incl. US, $5M medical, $250 deductible) a 65-year-old pays $210-259 for an 8-day cap, $251-309 for 15 days, and $292-360 for 30 days; age, destination and the medical limit move it from there. The number of trips you take does not change the premium.
Usually from the third trip — sooner if the trips are long. At 65 travelling to the USA a 14-day single trip costs $82-101, so three of them match the 15-day annual cap ($251-309). A 30-day single trip is $176-216, so two month-long trips already cost more than the 30-day annual plan ($281-346). If you travel once a year, single-trip cover is cheaper — do not buy annual out of habit.
Coverage stops at the cap: a 15-day cap on a three-week trip leaves the final six days with no emergency medical coverage outside Canada. Buy the cap that matches your longest trip of the year, or move that one trip to a single-trip policy.
No — standard annual plans cap every trip at 30 days. A winter in Florida or Arizona needs an extended or snowbird plan (typically 60-180+ days per trip) or a single-trip policy covering the whole stay. Compare those on our travel insurance for seniors page before buying a standard annual plan.
Only if the policy includes stable pre-existing condition coverage, and only when the condition has been stable for the period the policy names — commonly 90-180 days before departure. Cheaper plans without that clause will decline a claim tied to the condition, which for anyone on blood-pressure or diabetes medication is usually the wrong place to save.
Not with every carrier, and a lapse — even a single day — means travelling uninsured. Put the renewal date where you will see it and confirm the new policy is active before you leave. If a carrier does auto-renew, check that the per-trip cap and the medical limit are still what you need.
